Forms/RulesRules/Code Sections: BLR 9013-2 Filing RequirementsFiling Requirements: Document in basic captioned format - order Four-inch margin at the top of the first page only Include "END OF DOCUMENT" below final text of the proposed order DocketDocket: Ensure Filing Requirements are met Select Bankruptcy or Adversary Select Order Upload Select Single Order Upload Enter case number (xx-xxxxx) Click Next Enter the Related document number in the free-text box Click Next If the related document number field was left blank Select the category to which your event relates Click Next Select the appropriate event to which your event relates Click Next Verify the following before proceeding Case name and number Related document number Related document description Select the order type, using the drop-down box You will see the message: Note: If the related document is an amendment and the original document has been set for a hearing, then select order type of Hearing Scheduled Insert hearing date and time, if applicable Click the calendar icon to select date Click the clock icon to select time Click Next Browse to select the document to be filed (PDF) Review the order for accuracy before moving forward Click Next ‹ Request for Entry of Order Terminating Employer Deduction Order up Suggestion of Death ›
